Patient Referrals

Referrals are subject to an on-site evaluation conducted within 24 hours by an RML clinical liaison. Our clinical liaison nurses understand the sensitivities associated with moving a patient from a referring hospital to a long-term acute care hospital. They work closely with families and social workers to ensure that all aspects of the process run smoothly and appear seamless to the families.

Respectful of a patient’s privacy and the mandates of HIPAA, our nurses take special care to make recommendations for the patient and share information with the appropriate family members or authorized individuals. In addition, they can answer financial and insurance questions that may arise.

Please have the following information available so that your request can be processed efficiently:

  • Patient personal information (name, address, phone, other contact information)
  • Patient medical information (condition, cause, medical course)
  • Patient financial information (insurance, etc.)


Admission Criteria: Ventilator Weaning

Patients eligible for RML’s Ventilator Weaning program are typically new to the ventilator and show potential to wean. They may have the following diagnoses in acute states:

  • Chronic obstructive pulmonary disease (COPD)
  • Acute respiratory distress syndrome (ARDs)
  • Congestive heart failure
  • Muscular dystrophy
  • Pulmonary fibrosis

Physicians may also be treating a new diagnosis such as:

  • Guillain-Barre Syndrome
  • Pneumonia
  • Influenza
  • West Nile virus


Admission Criteria: Wound Management

Patients eligible for RML’s Wound Management program are suffering in an acute state with a severe wound management needs. They may have the following diagnoses:

  • Compression wound, possibly multiple Stage II-III or Stage IV with undermining
  • Dehiscence of a surgical site
  • Burns effecting a great amount of body surface areas
  • Acute infection
  • Sepsis
  • Osteomyelitis


Admission Criteria: Medically Complex

Patients eligible for RML’s Medically Complex program are typically suffering from a new injury that could include:

  • Traumatic brain injury (including, but not limited to, cerebral vascular accident, subdural hematoma, intracranial hemorrhage, anoxic episode)
  • Spinal cord injury
  • Post surgical with medical complications
  • Post vent wean with possible rehabilitation potential once medical issues are addressed
