Admissions and referrals

Patient Criteria

Patients eligible for RML's Outpatient Pulmonary program are not dependent on a ventilator and suffer from:

  • COPD
  • Asthma
  • Chronic bronchitis
  • Emphysema
  • Lung cancer
  • Lung transplant/reduction
  • Occupational and environmental lung disease
  • Pulmonary fibrosis

Refer a patient

    Have the following information available:
  • Patient personal information (name, address, phone, other contact information)
  • Patient medical information (condition, cause, medical course)
  • Physician name

ATTN: Intake Assessment Coordinator

    Include the following information:
  • Completed referral form*
  • Patient history and physical
  • Results of the mot recent pulmonary function tests
  • Other relevant reports such as chest x-ray, EKG, and stress test.
